Commercial Payer ERA Enrollment: Aetna, Cigna, UnitedHealthcare, Humana, Anthem and BCBS, Wellcare and TRICARE (2026)

Where each large payer sends you to enroll for 835 ERAs and EFT, the turnaround each one states, and the portal terms that decide whether a bot may ever touch the alternative. Quoted from payer sites.

The Short Answer

Every large commercial payer will send an X12 835 instead of making staff download remittances, but each one hands enrollment to a different intermediary: Optum for Aetna and UnitedHealthcare, Availity for Anthem, Humana and the BCBS plans, your EDI vendor for Cigna, PaySpan for Wellcare, PGBA for TRICARE East. The turnaround each payer states runs from 10 business days to six weeks. The second half of this article matters as much as the first: the portal terms of use, read on 2026-10-16, decide whether the portal download a practice is doing today may ever be automated. For most of these payers the answer is no without written approval, which is why enrollment is the route.

This is the commercial spoke of the ERA enrollment guide; the Medicare and clearinghouse spoke covers the MACs, Availity, Office Ally, Optum and Waystar. Every quotation below is from the payer's own page on the date given; anything not fetched is marked.

Aetna

Route. Aetna's ERA page: "ERA provides claim payment explanations in Health Insurance Portability and Accountability Act (HIPAA)-compliant files. To send an enrollment or change request, you'll need to register for ERA on the Optum® Payer Enrollment Services site. Both medical and dental providers can enroll. You don't need to enroll in ERA to get electronic EOBs." EFT goes the same way: "We won't charge you a fee for EFT, but your bank might. To send an enrollment or change request, you'll need to register for EFT on Optum's Payer Enrollment Services site." And a nudge: "If you don't enroll in EFT, we may issue future payments as a VCC." Turnaround and signer are not stated on the page.

Terms. Aetna's Digital Terms of Use (last updated June 24, 2026) say you may not "use any robot, spider, site search/retrieval application or other manual or automatic device to retrieve, index, 'scrape' 'data mine' or in any way gather the Services or reproduce or circumvent the navigational structure or presentation of the Services without our express prior written consent". Availity's own terms (April 1, 2026) carry no robot clause; Aetna's apply to Aetna's services.

Cigna Healthcare and Evernorth Behavioral

Route. Cigna's ERA instructions put the EDI vendor in the middle: "Notify your Electronic Data Interchange (EDI) vendor that you would like to enroll for Cigna Healthcare ERA. Provide enrollment information as instructed by your EDI vendor (if you use more than one TIN, complete a separate enrollment for each TIN). Your EDI vendor will send the completed enrollment information to Cigna Healthcare for processing; Cigna Healthcare will finalize your registration within 10 business days of receiving it." EFT is done in the provider portal: "Log in to CignaforHCP.com, select Working with Cigna, select Enroll in Electronic Funds Transfer (EFT) Options", after which Cigna sends a pre-note to the bank; its EFT page says "the enrollment process typically takes two to six weeks." Evernorth behavioral providers enrol EFT at Provider.Evernorth.com under My Practice, and "the enrollment process typically takes four to six weeks", with "a separate enrollment for each TIN". Evernorth's EDI page confirms the 835 "supports auto-posting capabilities" and points to Cigna's EDI vendor list.

Terms. Cigna's terms (version 2.2, June 4, 2026) prohibit access "through interfaces, robots, crawlers, scripts, or other automated means (unless approved in writing by us)", prohibit any attempt to "'scrape,' 'data mine,' 'harvest,' or otherwise aggregate, collect, or compile any data or other Site Content from the Site, whether manually or through automated means (unless approved in writing by us)", and add a clause for this decade: you may not "access or use the Site, or process any data or other Site Content from the Site, using any artificial intelligence model, tool, or capability (including without limitation large language models and generative AI)". Evernorth's terms (version 3.3, June 17, 2026) carry the same three clauses.

UnitedHealthcare and Optum Pay

Route. "You have 2 options for receiving payments from UnitedHealthcare. Both are facilitated by Optum Pay." For the 835: "835 files are received directly through your clearinghouse, or download them from the Optum Pay tool in the UnitedHealthcare Provider Portal. Contact your clearinghouse to request direct delivery of your UnitedHealthcare 835 files. Once we receive the request for 835s from your clearinghouse/EDI vendor, it takes about 30 days to set up delivery of the ERA/835." EFT: "The standard processing time takes about 10 business days." Pricing: "Basic: Available at no cost to health care professionals and facilities who enroll in ACH/direct deposit/EFT. Premium: Fee is 0.5% per ACH/direct deposit/EFT." Enrolment needs a W-9, a voided check or bank letter, and a named "TIN Super Admin". Routing is "completed at the TIN level", and a practice that does not enrol will "receive consolidated 835 files (a file for each check you receive) and continue to receive paper EOBs in the mail."

Terms. The UnitedHealthcare Provider Portal terms (effective October 20, 2025) say you will not "copy, modify or harvest data, Content, or materials from the Online Services" nor "use software or other means to access, 'scrape,' 'crawl,' or 'spider,' any web pages or other services from the Online Services. If you are blocked from the Online Services (including by blocking your IP address), you agree not to implement any measures to circumvent such blocking". Optum's own terms (April 21, 2025) carry the identical scrape, crawl and spider clause across Optum's sites and applications, which includes Optum Pay.

Humana

Route. Humana's electronic payment page: "Physicians and other healthcare providers can use Humana's ERA/EFT Enrollment tool on Availity to request enrollment or update an existing enrollment. To access this tool: Sign into Availity Essentials (registration required). From the Payer Spaces menu, select Humana. From the Applications tab, select the ERA/EFT Enrollment app." And: "Humana uses Availity as the central gateway for delivery of 835 transactions. You can access your ERA through your clearinghouse or through the secure provider tools available at Availity Essentials." Cost: "Fees may be associated with ERA transactions. Check with your clearinghouse for specific rates." Availity's Humana form adds a prerequisite: "In order to receive ERAs through Availity, you must be receiving your EFT directly from Humana." Turnaround and signer are not stated.

Terms. Not fetched. Humana's terms page returned only a JavaScript challenge to both a plain request and a headless browser, which is its own answer about automated access.

Anthem (Elevance) and the BCBS Pattern

Route. Anthem "uses Availity as its exclusive partner for managing all electronic data interchange (EDI) transactions." For ERA: "Log in to Availity. Select My Providers > Enrollment Center > ERA Enrollment. Note: If you use a clearinghouse or vendor, please work with them on ERA registration and receiving your ERAs." For EFT, since November 2021, "use enrollsafe.payeehub.org to register and manage EFT account changes", and a November 2025 provider notice calls EnrollSafe "our exclusive EFT partner". Turnaround is not stated.

BCBS Illinois shows the per-plan pattern. Its 835 companion guide (version 1.4): providers "registered with Availity Essentials must enroll online for ERA through Availity Essentials using Transaction Enrollment", and "this function must be performed by the provider's Availity Administrator." For a billing company: "To designate a billing service or clearinghouse as their designated ERA Receiver, the provider must either complete the enrollment process authorizing the vendor to retrieve their remittance files, or a copy of the Power of Attorney must be submitted at the time of enrollment." After approval "the provider will receive a letter from us to acknowledge and approve the request" with the start date, and "the paper PCS will be discontinued 31 business days from the date the provider begins receiving the ERA/EPS files." Out of state: "You should contact your local BCBS plan for 835 ERA enrollment. If an existing enrollment is already on file with the local BCBS plan, then ERA files for services provided to members of BCBSIL will be transmitted electronically to you under the Blue Cross Blue Shield Association's BlueCard program." EFT registration through Availity is free: "there is no cost to register."

Terms. Anthem's terms (effective and last reviewed October 1, 2026) prohibit "any robot, spider, site search/retrieval application, or other manual or automatic device, to retrieve, index, 'scrape,' 'data mine,' or in any way, gather information from this Application", and add: you will not "access or use any Application through an artificial intelligence enabled system, agent, bot, or other automated means, including technologies acting on behalf of a user or another party to submit inquiries, retrieve information, navigate functionality, or access account, member, provider, claims, benefit, or other information, unless expressly authorized by us in writing." BCBS Illinois's terms have no robot clause; the nearest is a ban on any "device or computer program that interferes or attempts to interfere with the operation of this website" and on copying or redistributing content.

Wellcare (Centene)

Route. Wellcare's North Carolina page: "Registering for PaySpan Health is quick and easy. Online enrollment takes about 10 minutes." Providers "enter their TIN, NPI and billing ZIP code as a prerequisite for EFT & ERA registration. A match on all three components will result in the ability to register without having to contact the payer or PaySpan Provider Services." Then "in a few days, you will need to verify with your bank that a minimal deposit has been made by PaySpan Health", after which "all subsequent payments will be sent electronically." Fees and signer are not stated on the page; state pages vary.

Terms. Wellcare's terms (October 1, 2026) have no explicit bot clause. They ban introducing a "spider, robot" or other foreign code into the site, any action that "imposes an unreasonable or disproportionately large load on the Site's infrastructure", and use "for any commercial or pecuniary purpose". The provider portal's terms and conditions carry credential and HIPAA terms only.

Molina Healthcare

Not fetched. Molina's ERA/EFT and terms pages returned only an application shell to a plain request, and a headless browser received a block page stating the request "looks similar to an automated request from a computer virus or spyware application". The route and terms are unverified here; ask Molina's provider services directly.

TRICARE East and West

TRICARE East (Humana Military, paid by PGBA). "The TRICARE East Region payer ID is 99727." "Providers who use clearinghouses to process their claims need to enroll via Electronic Data Interchange (EDI) express enrollment to process claims and set up EFT/ERA." The EFT and ERA package (form XPBB1124-B) is signed by the "person submitting enrollment" with "a bank letter or a voided check", and faxed or emailed to PGBA; there is a separate fax for "ERA enrollment only". Turnaround: "Please allow four weeks for the enrollment process to be completed and to begin receiving ERAs." "Payments are made at the NPI level." Humana Military's site terms carry no automation clause.

TRICARE West. Not fetched: tricare-west.com reset the connection on three attempts spaced minutes apart.

What the Terms Mean for a Practice

Read together, the terms sort these payers into three groups:

  • Written approval required for any automation: Aetna, Cigna, Evernorth, UnitedHealthcare, Optum, Anthem. Cigna and Anthem now name AI agents specifically.
  • No published automation clause: BCBS Illinois, Wellcare, Humana Military. Absence of a clause is not permission; it means the question has to be asked.
  • Site actively blocks non-browser access: Humana, Molina. A bot that works today on these will be the first thing their security team sees.

Opexia's commitment on this is fixed: We automate a payer portal only where the payer's terms of use allow it or the payer has approved it in writing, and we do not work around CAPTCHA, bot detection or two-factor login. Enrolling for the 835 is the route that needs no one's permission but the provider's.

Questions this article answers

Which payers enroll ERA through Availity?

Anthem calls Availity its exclusive EDI partner and routes ERA enrollment through My Providers, Enrollment Center, ERA Enrollment. Humana uses Availity as the central gateway for 835 delivery and has an ERA/EFT Enrollment app in its Payer Space. BCBS plans such as BCBS Illinois require online ERA enrollment through Availity's Transaction Enrollment, performed by the provider's Availity administrator.

Which payers enroll ERA through Optum?

Aetna sends both ERA and EFT enrollment to Optum's Payer Enrollment Services site. UnitedHealthcare's 835s are set up by asking your clearinghouse to request direct delivery, and payments run through Optum Pay, with a basic tier at no cost.

How long do commercial payers say ERA enrollment takes?

Cigna finalises ERA registration within 10 business days of receiving it from the EDI vendor, and says EFT typically takes two to six weeks. UnitedHealthcare says about 30 days for 835 delivery once the clearinghouse request arrives. TRICARE East says to allow four weeks. Evernorth behavioral EFT typically takes four to six weeks.

Can we automate the payer portal instead of enrolling for ERA?

For most large payers, not without written permission. Aetna, Cigna, Evernorth, UnitedHealthcare, Optum and Anthem each publish terms that prohibit robots, scraping or automated access, and Anthem's October 2026 terms add AI agents acting on a user's behalf. BCBS Illinois, Wellcare and Humana Military publish no such clause, but Humana's and Molina's sites blocked non-browser access outright during research.

What about BCBS plans in other states?

Enrollment is per home plan. BCBS Illinois tells out-of-state providers to contact their local BCBS plan for 835 ERA enrollment, and says an existing enrollment with the local plan carries over to BCBSIL claims under the BlueCard program.
